Hospital Announces Online Health Information Access Via Patient Portal

Humboldt General Hospital has announced that patients can now access their private health information via an online Patient Portal.

Humboldt General Hospital Clinical Informatics Coordinator Lupe Diaz said the YourCareCommunity website enables patients to access their personal health information easily, securely, and conveniently.

"Once patients have registered for the portal, they will be able to see their health records any time of day and from any location with an Internet connection," she said.

Both inpatients and outpatients can access a wide variety of key information from their health record such as lab results, prescribed medications, vital signs, diagnoses, discharge instructions, and more.

Additionally, Diaz said patients can view and change information related to their account, such as name, marital status, address and phone numbers, their password to YourCareCommunity, and more. They can also manage notifications, and add and manage patient representatives.

Plus, patients can download and upload a variety of health-related documents as well as find documents sent by their care providers.

Diaz said patients must initially sign up for the service at hospital registration, or through the HGH Medical Records department.

"You will need a personal email address," she said, "so come prepared with that information as well as photo identification."

Diaz said patients simply need an internet connection to access the portal.

While the portal includes all health information from Humboldt General Hospital, including the results from tests and studies performed in Humboldt General Hospital departments such as Laboratory and Radiology, the Patient Portal will not include information from any visits with HGH physicians and/or clinics, or non-HGH hospitals, clinics, providers or facilities.

"But as data becomes available from other sources in the future, we will add it to our patients' access," said Diaz.

In the meantime, Diaz said patients who need access to results that are not visible through the Patient Portal may visit Humboldt General Hospital's Medical Records department to obtain hard copies.

"Remember to register at Humboldt General Hospital," said Diaz, "but after that, you can access your health information from any internet connection in the world."
