Social Services

Humboldt General Hospital's Social Services Department helps patients and families deal with the unique concerns that illness and hospitalization can create.

Our team understands the impact of illness and related challenges on patients' and their loved ones' lives. As caring, dedicated professionals, we help people solve problems, strengthen their coping skills and recover from painful experiences. We also assist patients in communicating meaningfully with their health care teams and gaining access to hospital- and community-based resources.

Social Services

Our social worker is available for both inpatients and outpatients to help with issues such as:

  • Discharge planning
  • Community resources and referrals
  • Crisis intervention
  • Counseling
  • Assistance with Medicaid application
  • Assistance with Medicare Part D & Social Security application/resource
  • Long-term-care placement and referral
  • Facilitating support groups
  • Facilitating/chairing the HGH Ethics Committee
  • Psychiatric referrals and placements
  • Guardianship and Advanced Directive resource/referral
  • Coordinating the Telemedicine Clinic for Dementia screenings
  • Psychiatric/legal holds
  • Crisis counseling
  • Networking with community resources and knowledge of services available in this as well as outlying communities
  • Ongoing needs assessment
  • Guardianships of residents in coordination with Public Guardian's office
  • Working with SNF discharge planning as part of a team effort where applicable
  • Medicaid referral and information
  • Working as part of the supportive team in meeting resident/patient needs
  • Indigent services referral and information
  • Bariatric support resource and referral
